Health Services

The Fountain Valley School District recognizes that students and staff have the right to a safe and secure campus. Each school in the District is staffed with a part-time health clerk. Four District nurses oversee the medical needs of students within our CDC/preschool programs, our elementary campuses as well as our middle schools. Annually students in grades Kinder, 2nd, 5th and 8th grade undergo vision and hearing screening by qualified screening nurses.
Fountain Valley SD is set to comply with the new requirements of SB277, the law that requires all students enrolling after January 1, 2016 to have full immunizations prior to entering school.  This bill eliminates the personal belief exemption and requires full immunization or medical exemption as the only options.

Within 90 days of entrance into school, parents must provide a certificate documenting an oral health assessment and a physical examination prior to school entry.  Forms to complete with your medical professional are below.
If your child needs any attention regarding a Food Allergy, an Insect Allergy or has seizures, please fill out the appropriate forms below and return to your school.
